As an Outreach Youth Worker/Adolescent Health Specialist you will initiate contact and build relationships with youth experiencing or at risk of homelessness. You will provide collaborative street-based care with a partner and/or conduct group sessions to deepen relationships with youth, provide in-kind and other support and case management while assisting youth to achieve safe housing. You will use a harm reduction approach and provide supplies to street engaged youth while educating them on topics of central concern relating to basic needs, health, education and housing. You will be required to maintain records, database and documentation, complete program reports and participate in program related training and networking.

Job Requirements:
  • Bachelor's Degree in Social Work, Community Health, Youth Studies or related field plus 1 year experience working with youth or AA degree in related field plus 1 year experience working with youth and 2 years of additional relevant experience.
  • Experience in street outreach and youth work.
  • Knowledge of youth work that includes positive youth development, harm reduction and trauma-informed care.
